Office hours
are from 8:00 a.m. to 5:00 p.m. Monday through Friday. All visits are by appointment only.

When calling for an appointment:
	Call for an appointment several days in advance, when possible.
	Describe the nature of your problem to the person making your appointment.
	If you must cancel an appointment, please give us as much time as possible.

Our personnel will request that you bring any pertinent medical records or recent lab studies with you or have your physician forward these to our office. This enables us to review your medical history and may prevent duplicate tests.
